Our whakataukī "Ma te huruhuru ka rere te Manu”("without feathers, the Manu bird cannot fly") guides us to enable collaboration between care providers to support patient/whānau self-management in their aspirational pathways and goals.
We specialise in solutions to enable connected care in both the clinical and social care settings. Our vision is to unite multiple providers, and to put the patient/whānau at the centre of care.
Our purpose-built software platform and change management services make possible new service delivery models, which connect primary, secondary and community care providers, and catalyse better outcomes for all stakeholders.
We have public and private clients and delivery partners in New Zealand, Australia, Singapore, the United States and Canada.
Whānau Tahi’s parent organisation is Te Whānau O Waipareira Trust, an Urban Maori Authority that has been providing services locally and across Aotearoa for more than 30 years.
