A Hub of Wellbeing

Enabling Connections and Relationships that Lead to Wellbeing for All

We recognise that to realise the 360 degree vision of understanding whānau aspirations, goals, and cross-sector participation, it would be necessary to integrate Navigator with other systems.

One area which includes many different kinds of systems is the health sector. We provide the ability to link in secondary care providers (hospitals and specialists) as well as primary and community based health providers (general practice, pharmacy, dental, and community nursing) from a number of different platforms using our Connected Care platform.

We support that same holistic view for social service providers and whānau.

  • Whānau centred planning based on the whānau ora model of care. Includes whānau plans, assessments, referrals, group programs, contract management, dashboards and reports.
